Community Health Work Intern at Care Collab
Position Overview
We are seeking a Community Health Worker Intern to support our Outreach team in connecting prospective and newly enrolled members from priority accounts with essential health and social services. This role offers hands-on experience in community health work while developing expertise in healthcare systems navigation and resource coordination.
Essential Responsibilities
Direct Client Support (50%)
- Assist prospective and newly enrolled members in accessing medical services, social
programs, and community resources - Help members navigate complex healthcare and social service systems
- Provide warm handoffs between newly enrolled/prospective members and external
service provider + internal care management team - Support member in completing applications for benefits and services
- Accompany members to appointments when needed (virtually or in-person)
- Serve as initial point of contact for newly enrolled members requiring immediate
services
Resource Development & Management (30%)
- Develop and maintain comprehensive community resource directory
Create user-friendly guides for common healthcare and social service processes
- Document step-by-step navigation procedures for frequently used services
- Research eligibility requirements for various programs and services
- Track service availability and waiting lists
- Identify gaps in available resources and potential alternatives
Care Coordination (20%)
- Participate in team meetings with BDA and referral sources to understand service delivery
needs of prospective and newly enrolled members - Liaise between prospective and newly enrolled members, healthcare providers, and social services
- Follow up with providers and prospective and newly enrolled members to ensure
successful connections - Document all client interactions and resource referrals
- Track outcomes of service connections and referrals
- Identify and report barriers to accessing services

Required Qualifications
- Current enrollment in relevant degree program (Public Health, Healthcare
Administration, Community Health or related field) - Strong interest in community health and social services
- Excellent interpersonal and communication skills
- Basic computer literacy
- Ability to maintain confidentiality
- Cultural sensitivity and awareness
Preferred Qualifications
- Bilingual skills
- Previous volunteer or work experience in human services
- Familiarity with local community resources
- Experience working with vulnerable populations
Position Details
• Duration: 12-16 weeks (one semester)
• Hours: 15-20 hours per week
• Location: Hybrid (mix of remote and in-person work)
• Supervision: Weekly supervision with experienced Public & Community Health
professional
• Training: Comprehensive orientation and ongoing training provided
