Open Door is seeking candidates for a college internship in the Community Health Department.
Days and Hours: Monday-Friday 9am-5pm (flexible), minimum 20 hours/week for 3 months
Identify community resources and update resource lists such as o ABA (Applied Behavioral Analysis) o Developmental delays o Alzheimer’s and dementia o Clinical (Medical) referrals Assist with re-establishing linkage agreements with community partners
Assist with the identification of Health Home members (individuals currently receiving Care Management) missing Advance Directives
Assist with the outreach to and appointment scheduling for patients with chronic medical conditions needing a care plan (e.g. diabetes, hypertension)
• Excellent customer service skills
• Strong written and verbal communication skills
• Can work independently and follow detailed directions
• Open to learning and developing new skills
• Awareness of office etiquette and professional attire
• Bilingual (Spanish/English)
• Experience in health care preferred
